Rationale:
Clients with chronic conditions such as congestive heart failure should be further evaluated by the physician as to the cause of the fever. The nurse should not restrict the client's fluids. Providing warm blankets might make the client more uncomfortable. The client should not be encouraged to perform activity until the source of the fever has been identified.

Sedated patients should never be requested or allowed to sign an informed consent. If the informed consent has not been signed, the physician should be notified and the patient should not be allowed to undergo the procedure.
