Answer to Question 1

C
Secure carpeting would not be a safety hazard in the home environment. The curio cabinet would not be a safety hazard. Having night lights in the hall and bathroom is considered a safety feature and not a hazard. Scatter rugs on the kitchen floor can lead to slides and falls. The nurse should instruct the home care patient to remove the kitchen throw rugs.

Answer to Question 2

C
Specific criteria are used to determine whether the demonstrated behavior changes a patient makes are indicators of goal achievement. Standards are established before nursing action is implemented.
