Answer to Question 1

1. Explain to the father that the client is the only one who can obtain the results of the testing.
Rationale:
Since the client is an adult, she is the only person who is to receive results unless she signs a waiver and designates which others may receive the information. The nurse or physician may not breech the privacy of the client. It is not necessary to alert the client of the father's request for information.

Answer to Question 2

2. Provide a calm quiet environment and reassure the client that it is safe

Rationale:
The client diagnosed with post traumatic stress disorder who is exhibiting extreme anxiety needs immediate pharmacologic intervention, a quiet and calm environment, and reassurance of his or her safety. The client should not be given paperwork to complete. Asking the client what is causing the anxiety or encouraging the client to discuss what caused the syndrome to develop are not effective interventions for acute anxiety related to this disorder and should not be done.
