Answer to Question 1

C
An expected outcome is a description of the favorable patient status that is achieved when a prob-lem-focused nursing diagnosis has been resolved or positive adaptation has been enhanced.

Answer to Question 2

3. Ask the client what she needs to support her decision

Rationale:
The client's decision is to have an abortion even though it is against her religion. The best thing for the nurse to do is to ask the client what she needs to support her decision. The nurse should not provide information about adoption since the client is not planning on carrying the child to term. The nurse should not suggest she talk with clergy since the client did not ask to do so. The nurse should not remind the client that abortion is killing since this would not support the client's moral dilemma.
