Answer to Question 1

F
All activities that occur in the assessment phase are directed toward formulating the nursing diagnosis or identifying the problem.

Answer to Question 2

C
Older adults may take more medicines than those who are young and as a result are at risk for adverse drug reactions. Other problems with polypharmacy include medication errors, inappropriate prescribing, excessive drug costs, and noncompliance. Educating elderly patients and caregivers about medication, self-administration, and ways to increase compliance is a major nursing intervention.
