You charted the following nursing note using black ink: 8:00 a.m. ate breakfast; 7:30 a.m. prn medication given for pain; 10:00 a.m. bathed. What is incorrect about this entry?
 
  1. The prn medication should not have been recorded.
  2. The recording of the information is not done sequentially.
  3. Black ink is not the ink of choice for most institutions.
  4. The charting should have included the client's name.
