Answer to Question 1

D
Rupture of membranes usually occurs after labor has begun but can occur before the onset of labor. If labor does not begin spontaneously within 12 to 24 hours after rupture, labor may be induced to avoid infection. If engagement has not occurred, there is an increased risk of prolapsed cord. Nitrazine paper can confirm that the rupture is producing amniotic fluid.

Answer to Question 2

A, B, C, F
Enteral tube feedings are contradicted in clients with severe diarrhea, intestinal obstruction, peritonitis, paralytic ileus, and intractable vomiting. It should be used with caution in clients with severe pancreatitis, GI ischemia, and enterocutaneous fistula.
