Certain modifications are necessary to prevent bruising in the area of heparin injection. The nurse changes the needle after filling the syringe with the dose of heparin; that is, before injecting the client. The nurse does not aspirate the plunger once the needle is in place. Massaging the site is contraindicated because this can increase the tendency for local bleeding. The Z-track technique is used exclusively for IM injections.
