Answer to Question 1

A
Feedback:
In order to prevent self-absorption and contamination, the nurse should avoid touching the application with bare fingers. When applying the nitroglycerin paste, the nurse should remove one application before applying another and remove any residue remaining on the skin in order to prevent excessive drug levels in the client during application. The nurse should place the application paper on a clean, non-hairy area of skin as it facilitates drug absorption. The nurse rotates the site of medication to prevent potential skin irritation.

Answer to Question 2

C
Feedback:
When applying a transdermal patch, the nurse should be aware that a patch is applied to a skin area with adequate circulation. Most patches are applied to parts of the upper body such as the chest, shoulders, and upper arms. Small patches can be applied behind the ear. Each time a new patch is applied, it is placed in a slightly different location. The drug may still be active for up to 30 minutes after removal of the patch.
