The best proof of compliance with teaching standards is to document teaching and learning in the client's medical record. Asking the client or the physician does not prove that the teaching standards have been followed. Studying the change in the client's behavior is also not the best way to document the fact that the standards have been adhered to.

If the nurse plays with his or her hair during a client interaction, this can communicate insecurity. Superiority or confusion is less likely to underlie this form of nonverbal communication.
