When you give feedback, make sure to focus on things that are changeable, choose only one issue at a time, and give specific details. Feedback given should be specific regarding any mistakes that staff members make, explaining how to avoid the mistake or a better way to handle the situation. Saying, How can I trust you? and You are a bad assistant. are both derogatory and they do not tell what specific task was not done. Where did you learn to take vital signs is not something that can be changed.
