Answer to Question 1

C
Rest and sleep are affected by many factors, including degree of comfort, anxiety, environment, lifestyle, diet, drugs, age, and physical factors such as illness. The nurse can assess a client's altered sleep patterns by asking the client to rate the quality of his or her sleep.

Answer to Question 2

C
Parasomnia is a condition in which a person experiences profoundly disturbed sleep because of behavior or physiologic events. Common forms of parasomnia include somnambulism (sleepwalking), sleep talking, night terrors, rapid eye movement (REM) movement disorder, bed-wetting, and bruxism (teeth grinding). A nurse can assist the client to understand these conditions and to assess potential safety risks.
