Rationale: To a child, the tummy could be the abdomen, the stomach, or the chest. The nurse would need to get an accurate location, and giving the child a doll or a picture would be the best means of identifying the exact location. The child would not understand quality of the pain, and the parent cannot describe what the child is feeling, although he might be able to interpret certain behaviors. The child would interpret sharp as something pointy, and couldn't adequately answer this question.

Rationale: The mother is taught to dampen a cotton ball with normal saline or water to soften the secretions and make them easier to wipe away. The mother does not need to wear gloves, but should be taught to wash her hands before and after touching the child's eye. The mother then wipes from the inner to outer canthus, using a new cotton ball for each swipe.
