Answer to Question 1

D
Note that you do not include mention of the incident report in the patient's medical record. Instead you document in the patient's medical record an objective description of what happened, what you observed, and follow-up actions taken. It is important to evaluate and document the patient's response to the error or incident. Always contact the patient's health care provider whenever an incident happens.

Answer to Question 2

A
Clients prone to bleeding, such as the client with thrombocytopenia, must use an electric razor.
Clients with congestive heart failure may use a razor blade to shave.
Clients with osteoarthritis do not need to use an electric razor to shave.
Clients with pneumonia may use a razor blade to shave. If the client is wearing oxygen, an elec-tric razor should not be used, as it could create a spark (oxygen is flammable).
