Depending upon the issue, there may be several health care providers who may be a better choice. Proper cancer treatment will include a follow-up plan or survivorship care plan. The nurse will still advise the patient to call the health care provider if there are issues and to follow the instructions given, but acknowledging the team approach that is used and available support is most valuable to the patient at the time of discharge.
