Answer to Question 1

ANS: D
There are two primary causes of digoxin toxicity: improper dosing, and hypokalemia secondary to the concurrent administration of diuretics. The most obvious cause of toxicity is the admin-istration of too large a dose of digoxin.

Answer to Question 2

ANS: B
