A client with severe pernicious anemia tells the nurse, I do not understand why everyone is so excited about a little anemia. Lots of people are anemic.

The nurse responds that long-term pernicious anemia can cause severe damage to the tissues of the system?
  1. Nervous system
  2. Renal system
  3. Cardiovascular system
  4. Gastrointestinal system

Question 2

A client who had gastric bypass six months ago is profoundly anemic. The client has memory loss and periodic hallucinations. A diagnosis of folate deficiency has been proposed. Why would the nurse question this provisional diagnosis?

  1. Iron-deficiency anemia is the anemia that occurs among gastric resection clients.
  2. Folic acid absorption is usually enhanced during a rapid weight loss.
  3. Folate-deficiency anemia does not produce neurological symptoms.
  4. Folic acid deficiency has only a minimal impact on red blood cell counts.

Answer to Question 1

Correct Answer: 1
Rationale 1: Profound neurologic damage differentiates pernicious anemia from other anemias.
Rationale 2: Tissues of the renal system would not be affected to the degree of other tissues.
Rationale 3: Tissues of the cardiovascular system would not be affected to the degree of other tissues.
Rationale 4: Tissues of the gastrointestinal system would not be affected to the degree of other tissues.
Global Rationale: Profound neurologic damage differentiates pernicious anemia from other anemias. Tissues of the renal system, the cardiovascular system, and the gastrointestinal system would not be affected to the degree of other tissues.

Answer to Question 2

Correct Answer: 3
Rationale 1: Iron-deficiency anemia is not the only anemia that occurs among gastric resection clients.
Rationale 2: Folic acid absorption is not enhanced by weight loss.
Rationale 3: Neurological symptoms accompany pernicious anemia, but not folate-deficiency anemia.
Rationale 4: Folic acid deficiency has a major impact on normal red blood cell formation.
Global Rationale: Neurological symptoms accompany pernicious anemia, but not folate-deficiency anemia. Iron-deficiency anemia is not the only anemia that occurs among gastric resection clients. Folic acid absorption is not enhanced by weight loss. Folic acid deficiency has a major impact on normal red blood cell formation.

Once thought to have neurofibromatosis, Joseph Merrick (also known as "the elephant man") is now, in retrospect, thought by clinical experts to have had Proteus syndrome. This endocrine disease causes continued and abnormal growth of the bones, muscles, skin, and so on and can become completely debilitating with severe deformities occurring anywhere on the body.

A good example of polar molecules can be understood when trying to make a cake. If water and oil are required, they will not mix together. If you put them into a measuring cup, the oil will rise to the top while the water remains on the bottom.
