Answer to Question 1

C
After administering male perineal care for uncircumcised males, make sure that the foreskin is in its natural position. This is extremely important for those patients with decreased sensation in the lower extremities. Tightening of the foreskin around the shaft of the penis causes local edema, discomfort, and, if not corrected, permanent urethral damage. Assist the patient to a supine position and have him abduct his legs.

Answer to Question 2

C
C. The replacement schedule is determined by the type of lenses the client wears; the client could provide that information.
A. Tap water can contain microorganisms and may be absorbed into the lens, making it uncomfortable to wear..
B. Clean, powder-free gloves are used to prevent the spread of microorganisms during care of contact lenses.
D. Pain, tearing, discomfort, and redness can indicate conjunctivitis, and the client should be referred to his or her provider if symptoms persist.
