Answer to Question 1

D
D. End of fat emulsion tubing should be inserted into injection port of main IV, proximal to the venipuncture site, below the infusion filter on the main parenteral nutrition line.
A. Clean peripheral line tubing injection port of primary IV with alcohol swab. It is optional to use a Y connector.
B. Fat emulsions should hang no longer than 10 hours as a separate infusion. Fat emulsions may cause adverse symptoms if infused too rapidly as a separate infusion. Infusion time should be at least 4 hours.
C. Warm solution to room temperature if refrigerated. This prevents changes in body temperature from instillation of cold solution.

Answer to Question 2

D
D. If possible, turn the client on side, with head flexed slightly forward. Position prevents tongue from blocking airway and promotes drainage of secretions, thus reducing risk of aspiration.
A. Do not force any objects into client's mouth such as fingers, medicine, or tongue depressor, or airway when teeth are clenched. Prevents injury to mouth and prevents possible aspiration.
B. Do not restrain client. Loosen clothing. This prevents musculoskeletal injury and airway obstruction.
C. When seizure begins, position client safely. If standing or sitting, guide client to floor and protect head by cradling in nurse's lap or placing pillow under head. Clear surrounding area of furniture. If client is in bed, raise side rails, pad, and put bed in low position.
