Answer to Question 1

 4-line quatrain occurs within the first four measures of the usual twelve-bar blues stanza, one text line in each measure.

Answer to Question 2

 The quatrain is a b c d in which
a = I don't want to see Snook,
b = Not even Homesick James
c = The way my baby left me,
d = I really believe he's to blame.
 The refrain is A B in which
A = She got me walkin' all up and down the street
B = She left me for another man cause she wanted to be free.

People who have myopia, or nearsightedness, are not able to see objects at a distance but only up close. It occurs when the cornea is either curved too steeply, the eye is too long, or both. This condition is progressive and worsens with time. More than 100 million people in the United States are nearsighted, but only 20% of those are born with the condition. Diet, eye exercise, drug therapy, and corrective lenses can all help manage nearsightedness.
