Answer to Question 1

Any five of these:
Inability to lift head, smile, suck, or swallow
 Hard to wake up
 Dilated pupils
Pooling of blood in the eye
 Seizures
 Coma

Answer to Question 2

Failure to thrive can be a form of neglect directly caused by environmental factors, such as not feeding the child enough food, not attending to the child when he or she cries, and leaving the child alone for long periods of time with no interaction. These children appear emaciated, have a
vacant expression, and exhibit lack of energy.
