The patient is a 39-year-old female who injured her right ankle yesterday while walking around her garden. She apparently stepped into a gopher hole and suffered an inversion injury to her ankle. She complains of lateral ankle and foot pain, and has pain on weightbearing. On exam of her right lower extremity, she is nontender over the proximal tibia and fibula. The ankle shows moderate diffuse swelling on the lateral side extending onto the dorsolateral foot. Distal neurovascular status is intact. She has decreased range of motion secondary to pain and swelling. The diagnosis was acute right ankle sprain with avulsion fracture. A short leg splint was applied, she was given crutches, and she was instructed to be nonweightbearing and to ice and elevate the leg.
