Answer to Question 1

C
Feedback:
The nurse should step away from the bed when the physician says clear while defibrillating a client. The defibrillator discharges electricity, which can be conducted through the bed, and anybody in contact with the bed can be affected. The conductive gel pads are put in place before defibrillation. Shutting off the IV line and the equipment are incorrect nursing actions in this case.

Answer to Question 2

A
Medical management is aimed at clearing the airways and maintaining adequate oxygenation, using postural drainage and percussion; respiratory therapy with nebulizers or aerosol treatments; coughing and deep breathing; suctioning of the respiratory tract as needed; intravenous therapy as indicated; oxygen therapy as indicated; specific medications to treat the causative agent; and bronchodilators, mucolytic agents, and expectorants as indicated. Bed rest or limited activity is important, and small, nutritionally balanced meals should be offered at frequent intervals.
