Answer to Question 1

A, C, D
People use communication to fulfill basic human needs at all levels: physical, psychosocial, emotional, and spiritual needs. Communication is a process of sending and receiving messages. It forms the basis of sharing meaning and building effective relationships among individuals, families, and the healthcare team. Communication involves more than just talking and listening. Because messages are verbalized does not mean that listening and understanding are achieved.

Answer to Question 2

A
Impaired Verbal Communication is most likely the most appropriate diagnosis for the patient with expressive or receptive aphasia. In planning care, the best outcome is for the patient to develop other strategies and mechanisms for communication. Using alternative methods, such as writing and magnet boards, picture boards, or gestures, is an effective means of communication. Using these methods will assist in decreasing the frustration on the part of the patient and enhance communication between the nurse and patient.
