Failing to refill prescriptions because of the cost is typical for elderly who are on fixed incomes. Discuss the limitations that financial restrictions can place on this population and offer some suggestions for techniques that the health care team can use to emphasize the importance of the medications.

Answer to Question 1

Mrs. CL needs to take the medications prescribed for several weeks to give the stomach a chance to heal. During this time she must stay away from aspirin and follow her diet. These facts must be emphasized. Considering her relationship with her son and daughter-in-law, this must be emphasized to them also. By the end of the first prescription, her gastritis should be under control but additional time on medication would probably be necessary to prevent a recurrence.

Answer to Question 2

This patient may not be difficult to work with due to two main factors: 1 . she has a son and daughter-in-law who have been encouraging her to lose weight; 2 . they have already prepared a three-week cycle menu for her to follow. The care plan should include the son and daughter-in-law in consultation prior to and during the teaching of Mrs. CL. I would obtain a copy of the three-week cycle menu and modify it as necessary to meet Mrs. CL's needs. The care should include all of the principles listed in the answer to question 7 with emphasis on the small meals, low-fat foods, and chewing extremely well. Not eating while cooking is a problem also.
