The outcome criteria that were established during the planning step provide the basis for evaluation. During the assessment phase, the nurse is actively collecting data. Implementation is the phase of the nursing process during which the nurse performs interventions. Determining the needs of the patient and devising a plan of action take place during the planning phase.
