The nurse is working on a nursing unit that provides care for client's with Alzheimer disease. As the evening approaches, the nurse should plan which intervention for the clients in the middle stage of the disease?
 
  1. Lock the clients in their rooms.
  2. Restrain the clients in wheelchairs.
  3. Take the clients for a walk.
  4. Secure all doors leading out of the unit.

Question 2

The nurse is preparing to assess a client for vascular dementia. What should the nurse ask the client during this assessment?
 
  1. If the client has alterations of liver function
  2. If the client has experienced a head injury
  3. If the client has a history of smoking
  4. If the client has Parkinson disorder

1. Locking the clients in their rooms would be restraint of a sort and would provide safety, but is not appropriate.
2. Restraining clients in wheelchairs is not allowed.
3. Clients will middle stage Alzheimer tend to wander as night approaches. Taking the clients for a walk could risk their safety.
4. The clients should be able to freely move about the unit, but, the nurse should ensure that these clients are secured on the unit.

1. Hepatic encephalopathy is assessment for dementia caused by systemic diseases.
2. The client with a head injury is at risk for dementia, but the nurse is assessing vascular risks.
3. A history of smoking is a risk for vascular dementia.
4. Parkinson puts the client at risk for developing dementia from neurodegenerative disorders.
