The nurse should not ask the client if he enjoys crashing cars or ask why he had an accident today. The nurse should not report the client to the department of motor vehicles.

Before each feeding, the nurse should measure the abdominal girth to determine abdominal distention, which is seen in necrotizing enterocolitis or paralytic ileus.
