Answer to Question 1

Answer: D

The client is a repeat offender with motor vehicle accidents and would most likely benefit from a driver's safety course. This is what the nurse should suggest to the client. The nurse should not ask the client if he enjoys crashing cars or ask why he had an accident today. The nurse should not report the client to the department of motor vehicles.

Answer to Question 2

Answer: B

Before each feeding, the nurse should measure the abdominal girth to determine abdominal distention, which is seen in necrotizing enterocolitis or paralytic ileus. Weighing the baby and weighing diapers are interventions to assess for fluid volume status. Measuring pulse oximetry is an intervention for assessing oxygenation.
