Answer to Question 1

Correct Answer: 4

The nurse should replace the tube so that the medications can be administered in a timely fashion. Using coke to unclog the tube or ice water to irrigate the tube are not appropriate actions by the nurse. Tepid water should be used to irrigate the tube, if needed. Clients with nasogastric tubes are often unable to take anything by mouth, so administering the medications orally is not a realistic option.

Answer to Question 2

Correct Answer: 2

Although some facilities might have nurses calculate calories administered per day for clients with tube feedings, this is not routinely recorded with each feeding. All other information should be included following each feeding.
