Why is it a good idea to use a form to report progress on a patient?
 
  a. So none of the primary reporting items gets missed
  b. So you don't have to do as much writing/typing
  c. So you can copy the form and white-out the area needing changes, and then copy it again after making the updates
  d. For ease in billing
