Answer to Question 2

Answers should include:
The documented history provides information regarding the patient's past medical history, the review of systems and the history of present illness, all of which give insight into the professional relationship between the physician and the patient, as well as the patient's previous experience with the presenting problem.
Because the number of diagnosis reflects the presenting problem as either new or established, the documented history can provide clear indicators as to whether the physician is familiar with the presenting problem.
