The review of systems is an inventory of the patient's organ systems prior to the E/M service visit; this inventory of the patient's organ systems must specifically address the patient's status of health prior to the E/M service visit, which would be considered the past.
However, the past history identifies any medical condition, illness, injury, or treatment, which may be significant to the chief complaint, which is very different from an inventory of the patient's organ systems prior to the E/M service visit
