Answer to Question 1

2
Rationale:
1. The definition of health maintenance makes this an incorrect answer.
2. In health promotion, nurses partner with families to promote family strategies in the areas of lifestyle and coping.
3. This answer is simply a distractor, and does not relate to health promotion in families.
4. The definition of health supervision makes this an incorrect answer.

Answer to Question 2

A
At the beginning of the second stage, the cervix is completely dilated and effaced. Maternal pushing facilitates the fetal descent occurring during contractions. During the second stage of labor FHR and uterine activity are evaluated every 15 minutes for low-risk clients and every 5 minutes for high-risk clients. Transition occurs following the active phase; both of which are during the first stage of labor. This is too early for pushing. Pushing should begin at the beginning of the second stage.
