Answer to Question 1

ANS: A
An environmental assessment begins as the nurse leaves the agency en route to the client's home. The nurse should make specific observations. It would not be appropriate to use a computer or oral recording device while driving. Although the nurse should be conducting an environmental assessment, the nurse should be considering the needs of the client during this assessment and not his/her own needs. The nurse would not want to refer the client to another agency if the client has already selected this agency.

Answer to Question 2

ANS: B
Many FCNs find the need for further education to develop spiritual assessment skills, acquire theological knowledge, and learn the nurse's role in healing.

Chronic necrotizing aspergillosis has a slowly progressive process that, unlike invasive aspergillosis, does not spread to other organ systems or the blood vessels. It most often affects middle-aged and elderly individuals, spreading to surrounding tissue in the lungs. The disease often does not respond to conventionally successful treatments, and requires individualized therapies in order to keep it from becoming life-threatening.
