Answer to Question 1

B
Older adults with the most need for retirement planning are usually the same people who are least likely to receive it. The Mexican-American woman will most likely need preretirement counseling to avoid significant problems in retirement. She has three characteristics indicating a need for retirement planning: (1) her sex is female, (2) her job implies a lack of education or training, and (3) she does not pay into Social Security or federal taxes. Thus she can potentially fail to pay into the federal system for a sufficient length of time to be eligible for Social Security and Medicare benefits in retirement. In addition, her job is unlikely to offer a 401k plan.
Although the African-American woman is a member of a minority group, she is a profes-sional and likely to earn a significant income in a finance-related business. Her occupation and education places her well to receive significant retirement planning. The middle-aged man has one factor potentially associated with the need for retirement planningpoor health. The older clerk has one factor potentially associated with the need for retirement planninghe is likely to be a low-level employee.

Answer to Question 2

C

Feedback
A Incorrect. Encouraging fluids can be a reasonable nursing intervention for this woman; however, encouraging social activity without a complete assessment and without being in the environment of a trusting relationship is unlikely to help improve functioning.
B Incorrect. The husband can be displaying disenfranchised grief and be unable to help until his grief is managed.
C Correct. This couple is at high risk for chronic grief because of the nature of their son's death. The nurse can help this older female adult work toward better mental health by establishing a trusting and caring relationship with her to en-courage expressions about her son. Afterward, the nurse can pose probing ques-tions to determine the best plan of care to help this older adult resolve or reframe enough grief to allow improved functioning.
D Incorrect. A survivor's support group can be helpful to survivors of a loved one's suicide; however, expecting this older adult to attend such a group is unrealistic until the nurse establishes a trusting relationship with her.
