Answer to Question 1

C
A resident's blood pressure often decreases when he or she changes positions after being started on a psychotropic medication. Responses A and B, both vital signs, are important to monitor on all residents but are usually not affected by psychotropic medications. Therapeutic blood levels (response D) are not drawn until approximately 1 month after starting the medication.

Answer to Question 2

D
The Five Rights of Delegation include the right task, the right circumstance, the right person for the task, the right direction, and the right supervision. Patients may or may not influence delegation. Delegation can be organizational needs or tasks and patient care activities.
