Answer to Question 1

A
Sun exposure intensifies the normal aging changes and increases an older adult's risk of developing skin cancer. Responses B, C, and D all are personal choices for skin care. It is important to keep the skin clean and moist to maintain its body protection function; however, none of these practices alone affects the integrity of the aging skin to the extent that avoiding sun exposure does.

Answer to Question 2

C
Mrs. Gilbert has some obvious self-care problems. The greatest concern is her ability to meet her nutritional needs. Eating one meal a day probably contributed to her dehydration. Assisted ambulation and choosing to wear pajamas all day (responses A and D) are normal choices for older adults living in their own homes. The urine smell and the incontinence are concerns (response B); however, they are not life-threatening. Malnutrition is life-threatening.
