Decline: Emergence of sad or anxious mood as a problem that is not easily altered; emergence of an unplanned weight loss problem
Improvement: Decrease in the number of areas in which behavioral symptoms or sad or anxious mood are coded as not easily altered; overall improvement of resident's condition; resident receives fewer supports

Answer to Question 2

A significant change is defined as a change that normally will not resolve itself without intervention by staff or by implementing standard disease-related clinical interventions; impacts more than one area of the resident's health status; and requires interdisciplinary review and/or revision of the care plans.
