According to the Conditions for Coverage, how often must dialysis patients be seen by a physician, nurse practitioner, clinical nurse specialist, or physician's assistant providing ESRD care, as documented by a progress note placed in the medical record?

Question 2

At a minimum, how often should stable patients be reassessed and the plan of care revised?
