Some MCOs contract with the government to insure Medicare and Medicaid patients, and the MCO receives the premium directly from CMS or the individual states on behalf of these members.

The case manager focuses on the social and economic factors that interfere with a patient's ability to comply with the treatment plan. Case managers also assess cases for possible case management implications.
