It is not appropriate to convince the client that the facility itself is his home, because it is not the home he knows. The client should not be restrained, because this may cause the client to become combative or hostile. Comforting the client by stating that he will be sent home soon may not be of any help.

The nurse should assess for altered level of consciousness in a client with delirium. The client's level of consciousness can vary from extreme drowsiness to hyperactivity.
