The nurse should reassure the client that the facility where the client resides is safe. It is not appropriate to convince the client that the facility itself is his home, because it is not the home he knows. The client should not be restrained, because this may cause the client to become combative or hostile. Comforting the client by stating that he will be sent home soon may not be of any help.

The nurse should assess for altered level of consciousness in a client with delirium. The client's level of consciousness can vary from extreme drowsiness to hyperactivity. Complete loss of muscle control (akinesia), inability to eat or swallow (dysphagia), and inability to use objects properly (apraxia) are all seen in clients with dementia, not in clients with delirium.
