Answer to Question 1

C
Feedback:
The charge nurse is ultimately responsible for the total nursing care of clients while he or she is on duty.

Answer to Question 2

D
Feedback:
An aging adult's skin becomes dry, thin, and friable. Thus, the nurse should use bath oils instead of soaps when providing skin care to the client. This helps to keep the client's skin supple. Aging skin has very few oils and is fragile; daily baths should be avoided and lotions should be applied to the skin to prevent drying, irritation, and skin breakdown.
