The nurse need not assess what symptoms prompted the STAT order. The nurse assesses what symptoms have prompted the client to ask for PRN medication. STAT medications should be documented as soon as they are given and not before their administration. The nurse should document the date, time, and the nurse's initials next to the original order and not the client's initials.
