Factors influencing the location of the thoracic incision include physiologic intrapleural pressure changes and constant movement of the chest, adequate exposure into the thoracic cavity, maintenance of integrity of the chest wall and diaphragm, the condition of the underlying pleura and lung, and the objective of a minimally invasive procedure if possible.

The rigid scope can pass a biopsy or foreign body forceps and can allow oxygenation through perforations along the side of the tube.
