Axis III indicates any relevant general medical conditions. Axis IV reports psychosocial and environmental problems that may affect the diagnosis, treatment, and prognosis. Axis V is the GAF score.

This low GAF score indicates a consistent risk for self-harm exists; therefore the nurse's highest priority is safety.
