Answer to Question 1

A
If the assessment of a patient takes place under emergency situations such as at the scene of an accident or disaster, the nurse must remember that both Good Samaritan and abandonment laws apply. The nurse must render emergency care within the accepted standards of care and the nurse must stay with the patient until relieved by other qualified health care providers. If the nurse did not begin to give care or left the scene prior to adequate care being established, that nurse would be held liable for negligence.

Answer to Question 2

C
Hyponatremia is associated with headaches, apprehension, lethargy, confusion, depression, and convulsions. Hypernatremia is associated with restlessness, agitation, delirium, twitching, convulsions, and coma. Hypokalemia is associated with decreased bowel sounds, abdominal distention, paralytic ileus, nausea, and vomiting. Hyperkalemia is associated with nausea, muscle cramps, pain, and diarrhea.
