Answer to Question 1

B
Denial is the negation of reality of threatening situations despite factual evidence. Rationalization is the intellectual explaining away of threatening circumstances. Repression is the unconscious blocking from awareness of any material that is threatening or painful. Projection is attributing one's thoughts or feelings to another.

Answer to Question 2

A
The entire interdisciplinary team focuses on discharge planning but the social worker and the nurse have the greatest share of the responsibility. The social worker has knowledge of facilities and services. The nurse has knowledge of the patient's functional ability, cognitive and perceptual skills, and the understanding of the rationale for self-care skills. Working together, the nurse and social worker do the majority of the assessment and planning for the transition to another care facility or to home.
