Rationale: It is important to begin the interview in a manner that allows the nurse to gain the client's trust, develops rapport, and establishes a therapeutic relationship. Asking closed-ended questions will get one-word answers, and does nothing to foster rapport. Entering the information into the computer is a distraction to the client, and reassuring the client that she will be fine damages trust, because there is no way for the nurse to know this is true.
